While food allergies are more serious and generally easier to diagnose—you eat a peanut and everything swells up—food intolerances are subtler, with a more delayed reaction to a problematic food. This can make them more difficult to figure out, but still mean serious side effects, says Dave Rakel, M. D., the founder and director of the University of Wisconsin Integrative Medicine Program. If someone is intolerant to a particular food protein, like cow’s milk protein or gluten, eating it can “trigger an inflammatory response along the gut, that can lead to systemic or full- body inflammation,” Rakel tells SELF. Soy, eggs, shellfish, corn, and certain types of sugar are other common food triggers, he says, which can lead to gastrointestinal symptoms like diarrhea, constipation, bloating or abdominal cramping, or other issues like eczema, rheumatoid arthritis flare ups, and migraines, if someone has an intolerance. Anyone experiencing these symptoms is a good candidate for a medically- supervised elimination diet, Cimperman says. Those who just feel more tired or sluggish than usual probably aren’t—if that describes you, your better bet is to try cleaning up your diet by cutting down on sugary and processed foods and upping your vegetable intake to see if that helps. Because a doctor can run all the appropriate tests to get to the bottom of what’s going on, it’s important to embark on an elimination diet under their supervision. A doctor is also the only one who can order the appropriate test for patients who think they’re having issues with the wheat protein gluten, and make a celiac disease diagnosis. Anyone concerned about gluten should see a doctor before they remove it from their diet so that they don’t get a false negative from the necessary test, Cimperman says. Elimination diets can also lead to severe inadequacies in key nutrients, Cimperman says, which can be particularly problematic for people with diabetes, children or the elderly. A dietitian can help find substitutes for eliminated foods to ensure that you’re getting all the nutrition you need, says Largeman- Roth. That might mean replacing calcium someone was getting from milk with calcium from dark leafy greens or other non- dairy sources, recommending nutritious and gluten- free grains, or suggesting soy- free sources of protein. There are different types of elimination diets. Medically- supervised elimination diets are specific to each individual, which means that while one person might be put on a diet that eliminates gluten and dairy, another person might eliminate soy and certain types of sugar. Both are trying to determine which food proteins and sugars are causing one’s problems. For people with irritable bowel syndrome, a specific type of elimination diet that focuses on removing fermentable sugars instead of food proteins, called a low FODMAP diet, is often prescribed, Rakel says. In people with IBS, these sugars can produce excess gas, which can cause bloating and abdominal pain, and draw too much water into the gut, which can cause diarrhea. Removing them from one’s diet and then only reintroducing those that are tolerated can lead to a real improvement in IBS symptoms, Rakel says. They all involve a LOT of work. Be prepared to keep a detailed food and symptom diary both before you get started and throughout the two- to eight- week process, Cimperman says. This time period includes at least two weeks to eliminate foods—long enough to ensure that any potential food triggers are out of the system—followed by an additional few weeks to reintroduce potentially problematic foods one at a time to test the body’s response. She remembers having a hard time deciphering food labels, but that should be much easier today, because of food labeling laws. Finally, before you embark on an elimination diet, consider your lifestyle, Rakel says. They’re not guaranteed to ease your symptoms. While Rakel says he’s seen great results from elimination diets, specifically in patients with IBS or rheumatoid arthritis, in his experience, they don’t work for everyone. Elimination diets only uncover a food intolerance that, once removed, leads to improved symptoms about one- third to one- half of the time, he says. For this rest of his patients, he recommends a healthy, balanced and anti- inflammatory diet, but “food isn’t their problem.”6. But when they work, they can come with long- lasting and sometimes surprising benefits.“Elimination diets are a really useful tool to get to the root of what might be stimulating dysfunction in the body,” Rakel says. They’re also less likely than medications to cause long- term harm. Rather than jump straight to a medication to control heart burn or gastrointestinal symptoms, he says he prefers this more natural and in many cases more sustainable approach. For Largeman- Roth, an elimination diet actually helped her expand her food horizons, and she thinks it can do the same for others. When her diet coincided with a San Francisco trip, she was bummed to skip out on the sourdough, she says, but despite being a vegetarian, decided to try some seafood. You may have to repeat the elimination diet at some point. Food allergies and intolerances can change with age or because of a pregnancy or other environmental factor, experts say. This means that just when you think you have your intolerances pinned down, they can shift, or a new allergy can crop up. Largeman- Roth says that while she knows she can’t tolerate sugar alcohols and can only eat small amounts of soy, a couple of years ago—and seemingly out of the blue—her symptoms starting flaring up when she consumed hibiscus. Now she steers clear of that, too. On the other end of the spectrum, Rakel says that once the health of the gastrointestinal system is improved, people can sometimes learn to tolerate foods, at least in small doses, that they once had to cut out of their diet. Building up this tolerance can take three or four months, he says, so “people have to be patient,” but the potential upside—a balanced and complete diet and a healthy gut—is huge. The result is an increase in blood glucose levels, ensuring a supply of fuel in anticipation of the wakening body’s needs. If you take insulin injections, it could be that the effect of insulin you took is waning. Your blood glucose will rise if you didn’t take enough to keep your insulin level up through the night.
